Required Documents

1. Completed Claim Form including bank details

2. Comprehensive Veterinary Report containing:

• • • Previous history including details relating to onset of symptoms
    • Post mortem & autopsy report

3. Justification/proof of value:

• • • Purchase receipt or invoice
    • Contract of Sale
    • Letter from independent trainer, breeder or judge

The above documents are required in order for your claim to be assessed. If they are not provided it may lead to a delay in assessing your claim.

a. If the claim is accepted, Affinity will settle the claim to you minus the applicable excess.
b. If further information is required Affinity will contact you.

4. For ongoing vet treatment, provide further invoices and reports to Affinity for assessment by the insurer.

1. Completed Claim Form including bank details

2. Itemised Vet Invoices

3. Comprehensive Veterinary Report containing:

• • • Full registered name of horse (as noted on your policy schedule)
      and date of birth, sex and colour
    • Previous history including details relating to onset of symptoms
    • Treatment provided and future treatment required
    • Details of injury including causation
    • Future Prognosis

The above documents are required in order for your claim to be assessed. If they are not provided it may lead to a delay in assessing your claim.
